All patientsYour Medicare card, pension card and private health insurance details (if any) Details of all doctors involved in your care (including their addresses/contact details) Details of all the medications you take Test results - ie carotid doppler, CT scans, chest x-rays, echocardiogram

Patients referred for heart surgeryA detailed referral from your Cardiologist Details of imaging you have, ie a disc or memory stick with the details of your cardiac catheterization/angiogram. If you don’t have a copy, we need to know where these tests were done so we can access them electronically or obtain a copy.

Patients referred for lung/thoracic surgeryA detailed referral from your thoracic physician or GP Details of imaging you have had done either in the form of x-rays, a memory stick containing the images or details of where the imaging was done so we can access electronically.

Patients referred for pacemaker or defibrillator surgeryA detailed referral from your Cardiologist or GP Report of your last device check (if applicable)
The patient journey
Following your consultation with one of our specialist doctors you will generally be on the path to surgery.
If you are a “country” patient, to save multiple expensive trips to Sydney, we try and organise a ‘”one-stop shop” so that you come to Sydney, see the specialist, and then have your pre-admission tests, followed by surgery, all in one trip.
